Immigration Medical Examination.
General Information
When you apply for a green card (adjustment of status) in the United States, you are required to undergo an immigration medical examination. The examination must be done by a doctor who is designated by U.S. Citizenship and Immigration Services (USCIS). USCIS designates certain doctors (known as civil surgeons) to perform the immigration medical examination required for most green card applicants.

What Does an Immigration Exam Include?
- Completion of the required USCIS I-693 form
- Physical examination
- Vaccination history review
- Screening for tuberculosis, syphilis*, and gonorrhea*
* in addition to exam, it is required for adult
Fees: $170 for Adult or Child plus $130 for screening tests as required by USCIS (for 15 yrs and older).

If further testing or vaccinations are required to fulfill the Immigration application process or address an identified concern, MyHealth Urgent Care offers the convenience of blood work, x-rays, and vaccinations during the same visit. Additional fees will apply.
Remember to bring the following to your appointment:
- I-693 Form-Part 1 can be completed; DO NOT SIGN Part 2 (Download the Form)
- A valid government issued photo ID (passport, ID card, or driver’s license)
- Immunization records (official English translation)
- Payment, needed at time of visit (cash, check, or charge)*
- An Interpreter who can speak, read and write in English if needed.
